下丘脑性闭经下丘脑性闭经 下丘脑及中枢功能紊乱引起的闭经,是继发性闭经最常见的原因。中枢神经系统-下丘脑功能失调可影响垂体,进而影响卵巢功能导致闭经。如精神创伤、环境变化等因素可扰乱中枢神经与下丘脑间的功能,导致排卵障碍。中枢神经对体重急剧下降极为敏感,不论是单纯性体重下降或是真正的神经性畏食,体重低于标准体重的15%~25%时,可诱发闭经。剧烈运动可能因脂肪过度减少或因GnRH的释放受抑制,而导致闭经。某些药物如奋乃静、氯丙嗪、利血平及甾体类避孕药,可能抑制下丘脑分泌GnRH或通过下丘脑抑制PIF或多巴胺的释放,使PRL升高而导致泌乳和闭经。多囊卵巢综合征,由于下丘脑-垂体-卵巢功能失调,以致排卵障碍而闭经。其他如肾上腺、甲状腺功能紊乱也可引起闭经。
