镉污染对健康的影响  镉是对人体有害的元素,在自然界中多以化合态存在,含量很低,大气中含镉量一般不超过0.003微克/米3,水中不超过 10微克/升,每公斤土壤中不超过0.5毫克。这样低的浓度,不会影响人体健康。镉常与锌、铅等共生。环境受到镉污染后,镉可在生物体内富集,通过食物链进入人体,引起慢性中毒。    污染来源  20世纪初发现镉以来,镉的产量逐年增加。相当数量的镉通过废气、废水、废渣排入环境,造成污染。污染源主要是铅锌矿,以及有色金属冶炼、电镀和用镉化合物作原料或触媒的工厂。镉对土壤的污染主要有气型和水型两种。气型污染主要来自工业废气。镉随废气扩散到工厂周围并自然沉降,蓄集于工厂周围的土壤中,可使土壤中的镉浓度达到40ppm。污染范围有的可达数公里。水型污染主要是铅锌矿的选矿废水和有关工业(电镀、碱性电池等)废水排入地面水或渗入地下水引起的。    危害和机理  进入人体的镉,在体内形成镉硫蛋白,通过血液到达全身,并有选择性地蓄积于肾、肝中。肾脏可蓄积吸收量的1/3,是镉中毒的靶器官。此外,在脾、胰、甲状腺、睾丸和毛发也有一定的蓄积。镉的排泄途径主要通过粪便,也有少量从尿中排出。在正常人的血中,镉含量很低,接触镉后会增高,但停止接触后可迅速恢复正常。镉与含羟基、氨基、巯基的蛋白质分子结合,能使许多酶系统受到抑制,从而影响肝、肾器官中酶系统的正常功能。镉还会损伤肾小管,使人出现糖尿、蛋白尿和氨基酸尿等症状,并使尿钙和尿酸的排出量增加。肾功能不全又会影响维生素D3的活性,使骨骼的生长代谢受阻碍,从而造成骨骼疏松、萎缩、变形等。慢性镉中毒主要影响肾脏,最典型的例子是日本著名的公害病──痛痛病,但日本的痛痛病有明显的地区性,其中某些症状(如骨软化)在其他发生慢性镉中毒地区比较少见。慢性镉中毒还可引起贫血。急性镉中毒,大多是由于在生产环境中一次吸入或摄入大量镉化物引起的。大剂量的镉是一种强的局部刺激剂。含镉气体通过呼吸道会引起呼吸道刺激症状,如出现肺炎、肺水肿、呼吸困难等。镉从消化道进入人体,则会出现呕吐、胃肠痉挛、腹疼、腹泻等症状,甚至可因肝肾综合症死亡。    从动物实验和人群的流行病学调查中发现,镉还可使温血动物和人的染色体发生畸变。镉的致畸作用和致癌作用(主要致前列腺癌),也经动物实验得到证实,但尚未得到人群流行病学调查材料的证实。    预防措施  镉一旦排入环境,它对环境的污染就很难消除,因此预防镉中毒的关键在于控制排放和消除镉污染源。对慢性镉中毒患者,用维生素D和钙剂等进行治疗。如用络合剂驱镉治疗时,必须慎重,应从小剂量开始。中国《生活饮用水卫生标准》规定镉含量不得超过0.01毫克/升,《工业企业设计卫生标准》规定地面水中镉最高容许浓度为0.01毫克/升;《工业"三废"排放试行标准》规定废水中镉含量不得超过 0.1毫克/升。
