头孢菌素类抗生素头孢菌素类抗生素cephalosporins分子中含有头孢烯的半合成抗生素。曾译先锋霉素。β-内酰胺类中的又一大类抗生素,抗菌谱广(除革兰氏阳性菌外,对一部分阴性菌也有效),较稳定(分解较青霉素慢),过敏反应发生率较低(但与青霉素的过敏为部分交叉,如作皮试,必须单配皮试液)。但它的抗菌谱广是相对的,并有一定肾毒性,中枢神经系的反应和出血倾向也是值得注意的问题。临床所用的头孢菌素可分为3代:第一代以抗革兰氏阳性菌为主,也能抗一部分阴性菌,如头孢唑啉(先锋Ⅴ号)、头孢氨苄(先锋Ⅳ号)、头孢拉定(先锋Ⅵ号)等;第二代对革兰氏阳性菌的作用与第一代相似,但抗阴性菌的作用优于第一代,如头孢羟唑;第三代的抗阳性菌作用也与第一代近似,它除对大部分阴性菌有较强的抗菌作用外,还有较强的抗绿脓杆菌作用是其特点,如头孢哌酮、头孢他定(复达欣)等。①头孢唑啉(先锋Ⅴ号)。为半合成的第一代头孢菌素,除对革兰氏阳性菌有作用外,还对多种致病的阴性杆菌如大肠杆菌、奇异变形杆菌、克雷伯氏杆菌、流感嗜血杆菌等均有较强的作用。对β-内酰胺酶的耐性较弱。注意:对注射局部有刺激;可致转氨酶升高及白细胞减少,也能引起药物热等过敏反应;供肌注之针剂不可供静注用;有一定肾毒性。②头孢氨苄(先锋Ⅳ号)。为半合成的第一代口服头孢菌素,除对革兰氏阳性菌有抑制作用外,对革兰氏阴性杆菌也有作用。需注意,会偶有胃肠道反应;肾功能不良者,应适当减少用量。
