软骨营养不良  一种始于胎儿的软骨发育障碍,以长骨受累最重,表现为四肢与躯干比例失常的侏儒症。其基本病理改变是骨骺板不能产生足够的柱状软骨细胞,膜下成骨正常而软骨内成骨障碍。又称软骨发育不全。一般认为是常染色体显性遗传。多为散发病例。母亲年龄越大所生小儿发病率越高,男女发病率无差别。    多数患者出生时体征即很明显,有骨骼畸形头大,面宽,额前突,鼻梁扁平,下牙槽突起,下颌骨突出,四肢粗短,上臂及股骨最明显,躯干相对较正常。可有鸡胸和肋骨外翻。指短而分开。腹膨隆而臀后翘。轻度者在新生儿期不易察觉,面貌基本正常,身高不超过140cm,出牙正常。智力无障碍,周身肌肉发达。皮肤软、松弛。因骨骼异常而步态摇摆,一般内分泌及性功能正常。骨骼 X射线摄片可见头颅大,后颅凹小,蝶鞍扁宽,长骨粗短,骨骺扁平并有毛刷样变化。骨厚和骺端不规则是本病X射线基本特点。    本病需与能引起侏儒畸形的其他疾患鉴别:克汀病伴智能低下。先天性成骨不全症,长骨细长与本病中长骨粗短完全不同。佝偻病有其临床特点,X射线片示骨骺钙化不全,而本病虽骨骼有类似形态改变但钙化良好。另外还要与粘多糖贮积病及垂体性侏儒症相区别。    目前对该病无特殊治疗。垂体前叶激素治疗有一定疗效,近年有试用腮腺素治疗。股骨胫骨的内翻畸形可用矫形手术治疗。预后随病变严重性而异,大多数患儿死于胎儿或新生儿时期,存活者一般健康良好,智能正常,少数患者成年后因椎骨狭小或多发性椎间盘突出压迫脊髓和神经,引起背痛、坐骨神经痛等神经系统并发症。女性患者怀孕后因骨盆畸形致难产。
