细针穿刺细胞学检查细针穿刺细胞学检查 简称细针穿刺。即用22号针头连接10~20ml注射器直接穿刺肿块,以负压吸出细胞,进行细胞涂片检查。用于良、恶性肿瘤的鉴别,及早期发现和诊断转移和复发的肿瘤。此法于1956年欧美先用于诊断前列腺、淋巴结、乳腺、甲状腺肿瘤。1970年后开始用于妇科肿瘤的诊断,根据肿瘤生长的不同位置可:①经皮穿刺;②经阴道或直肠穿刺;③经腹壁穿刺。本法简单安全,但如针吸部位不准确,或针吸仅局限在一个部位,所吸标本不能代表整个肿瘤的性质,影响诊断的准确性。故盆腔包块如已考虑新生物或不能排除新生物而有手术适应证的包块,则不必行细针穿刺作细胞学检查,尤其是卵巢肿块,即使是良性也可能有小乳头,一旦溢出进入盆腔,也可能发生良性乳头腹膜广泛种植。
