血气分析血气分析bloodgasanalysis血中酸碱度、有关溶解在血中的氧及二氧化碳气体的检测。已在临床广泛应用。它可了解体内酸碱平衡和氧代谢情况,在危重病人的抢救中,是重要的监护手段。常用的血气分析的项目有以下几项:①pH值。pH是表示酸碱度的指标。正常血液的pH值为7.35~7.45。血液pH的恒定是依靠血浆中的缓冲体系,包括碳酸与碳酸氢盐系统、磷酸二氢盐与磷酸氢二盐系统以及蛋白质系统。②二氧化碳分压(PCO2)。指血浆中溶解的二氧化碳的张力。正常情况下动脉血内PCO2与肺泡内PCO2构成平衡关系,故动脉血的PCO2即可反映肺泡通气功能。PCO2的正常值为35~45mmHg(4.67~6.0kPa),小儿偏低为34~40mmHg(4.53~5.33kPa),这与小儿新陈代谢率较高、呼吸频率相对快有关。③标准碳酸氢根(SB)和实际碳酸氢根(AB)。在正常人,血浆中测得的AB与SB一致。有呼吸因素参杂时,则二者有差别。SB的正常值是22~27毫克分子/升,平均24,此值可反映体内代谢因素的改变。二氧化碳结合力(CO2CP)是另一种反映体内碳酸氢根含量的一种方法。正常值为50~70容积%,若需转换成毫克分子/升,则可除以2.2。如SB和AB一样,CO2CP也只能反映代谢性酸碱紊乱。④缓冲碱(BB)和剩余碱(BE)。BB指体内能中和酸性物质的阴离子,主要是碳酸氢盐、蛋白质和磷酸氢盐。三者总和为42毫克分子/升。BE表示血浆或全血的碱贮备增加或减少的情况。BE在碱中毒时为正值,说明体液内碱过多,在酸中毒时为负值。正常值为±3毫克分子/升。BE是观察代谢性酸碱平衡比较直接和可靠的指标。⑤氧分压(PO2)。指物理溶解在血浆中的氧气的分压。动脉血氧分压的正常值为80~100mmHg(10.66~13.33kPa);老年人>70mmHg(9.33kPa)即为正常。静脉血氧分压为40mmHg(5.33kPa)。⑥血氧饱和度(SO2)。指血红蛋白含氧的百分数,正常SO2为95%~98%,静脉血SO2为75%。血气分析标本采集用肝素化注射器取动脉血,或用毛细血管动脉化方法取血后,用血气分析仪测定。血气分析的结果受技术因素、仪器因素、测试时间、年龄、地区及病情变化等影响。对血气分析的结果要结合临床及标本采集过程、仪器测试条件进行全面分析。
