蛋白尿蛋白尿proteinuria尿中血清蛋白超过正常范围的表现。正常人每日约有180升血浆自肾小球滤过,但99%以上的蛋白质被肾小管重吸收,所剩蛋白质及肾小管分泌的极微量粘蛋白等构成正常人的蛋白尿,每日排出量不超过150毫克,常规检查法不能测出,其组成包括40%的白蛋白、50%的其他血浆蛋白,来自肾脏和泌尿生殖系统各部的组织蛋白占40%,15%为免疫球蛋白及其片段。肾小球滤过膜结构变化及肾小管回吸收和排泌变化均可使尿中出现过多的蛋白。依尿中蛋白质含量不同而测量结果分别为±、+、++、+++、++++,分别相当于100毫升尿中约含蛋白质30毫克、50毫克、100毫克、300毫克及超过300毫克。蛋白尿可分为生理性及病理性两种。生理性蛋白尿为一过性的,多见于高热、剧烈运动、情绪激动、寒冷刺激以及因脊柱前凸位致使肾静脉淤血造成的直立性蛋白尿。病理性蛋白尿由于病因及发病机理不同,大致分为肾小球性蛋白尿、肾小管性蛋白尿、溢出性蛋白尿及肾组织性蛋白尿。
