钙磷代谢钙磷代谢calciumandphosphatemetabolism钙和磷在食物中被机体所摄取,然后在体内进行合成和分解,以及最后被排出的全部过程。钙、磷在体内的含量与分布钙磷主要以无机盐形式存在体内。成年人体内钙总量约占体重1.5%,即700~1400克;磷的总量约为400~800克。约99.7%以上的钙与87.6%以上的磷以羟磷石灰3Ca3(PO4)2、Ca(OH)2的形式存在于骨骼和牙齿中。血浆中的钙每100毫升仅8.5~11.5毫克,以游离钙,或与其他离子结合、或与血浆蛋白结合的方式存在。血浆中的磷以无机磷酸盐的形式存在,成年人的含量为每100毫升约3~4.5毫克。正常人血浆中钙与磷的浓度维持相对恒定,当血磷增高时,血钙降低。当血钙增高时血磷减少。此种关系在骨组织的钙化中有重要作用。钙、磷的吸收与排泄体内的钙主要来自食物,大部分在小肠的上段吸收。钙的吸收量与肠道内钙浓度、机体的需要量及肠内酸碱度有关。摄入的钙80%从粪便排出,20%从肾排出。从肾小球滤过的钙有98%被重吸收。磷的吸收部位也在小肠的上段。当肠内酸度增加时,磷酸盐的吸收增加。钙、镁、铁等离子的磷酸结合成不溶性盐时,不易吸收。故当血钙升高时肠内钙浓度增加,从而妨碍磷的吸收。摄入的磷从粪与尿中排出,后者占60%。血浆钙、磷水平的调节每日钙、磷的摄入量与排泄量取得动态平衡,血钙、血磷水平维持相对稳定,这有赖于3种激素的协同作用,即甲状旁腺激素、降钙素及1,25-二羟胆钙化醇〔1,25-(OH2)-D3〕。①甲状旁腺激素。是调节血钙水平的主要内分泌素。切除甲状旁腺后,血钙很快下降。血钙水平对甲状旁腺激素的分泌起负反馈的调节作用,当血钙超过每100毫升12毫克时,甲状旁腺激素的分泌极少;反之,当血钙低于每100毫升4毫克时,甲状旁腺激素的分泌水平达高峰。甲状旁腺激素对肾脏排磷的调节作用较对钙重吸收的调节更明显。②降钙素。甲状腺的滤泡旁细胞(C细胞)分泌一种可降低血钙水平的激素,叫做降钙素。降钙素的生理作用是降低血钙与血磷的水平,它可以直接抑制骨质溶解,使释放入血的骨盐减少,同时骨骼仍继续从血浆中摄取钙,从而起到降低血钙和血磷水平的作用。还可抑制肾小管对钙、磷的重吸收,因而使尿钙尿磷增加,而使血钙血磷减少。降钙素抑制肠道对钙的吸收是间接的。此外,1,25-二羟胆钙化醇对血浆钙磷也有一定的调节作用。
