甲状旁腺激素甲状旁腺激素parathyroidhormone甲状旁腺主细胞分泌的碱性单链多肽类激素。简称PTH。甲状旁腺激素是由84个氨基酸组成的,它的主要功能是调节脊椎动物体内钙和磷的代谢,促使血钙水平升高,血磷水平下降。在甲状旁腺主细胞内首先合成PTH的第一前身物质,称为前甲状旁腺激素原,含115个氨基酸,以后这一前身物质在细胞内裂解成为含90个氨基酸的第二前身物质甲状旁腺激素原,后者进而在细胞内裂解成为含84个氨基酸的多肽,即PTH。正常人血浆中PTH的浓度约为1纳克/毫升。PTH在循环血液中的半衰期约为20分钟,主要在肾脏内灭活。体内钙代谢虽然受多种激素的影响,但是调节细胞外液中钙离子浓度的两种主要激素是PTH和甲状腺滤泡旁细胞分泌的降钙素。此外,PTH还间接促进肠道对Ca2+的吸收。PTH对肾脏的直接作用是促进肾小管对Ca2+的重吸收,因而减少Ca2+从尿中排泄。PTH通过活化维生素D3间接使肠道吸收的Ca2+增加。PTH的分泌主要受血浆Ca2+浓度的调节。如果PTH的分泌过于旺盛,骨形成与骨消蚀的平衡遭到破坏,被增强的破骨活性占优势,长期下去会引起骨钙质的消蚀而易于骨折或骨畸形,并因血钙量过高而导致一系列恶果。若PTH的分泌不足,肾脏的磷酸盐排泄量减低,磷酸钙沉积于骨。调节体内钙代谢的另一重要激素是降钙素,它是由甲状腺滤泡旁细胞分泌的一种肽类激素。降钙素可以降低血钙水平。
