病毒性皮肤病 病毒感染引起的、主要表现为皮肤粘膜损害的疾病。较常见。病毒感染后也可通过免疫机制引起病毒疹。 病毒感染皮肤与粘膜有3种途径：①直接接种,如各种疣、挤奶人结节等。②系统性感染，如麻疹、风疹、水痘等。病毒进入人体后，形成病毒血症，继之侵入皮肤并增殖，最后导致皮疹发生。但还有一些皮疹的发生可能由于皮肤对病毒抗原的变态反应。③内部病灶的局部播散，如带状疱疹。带状疱疹与水痘均为水痘－带状疱疹病毒引起。此病毒的原发感染为水痘，而复发感染为带状疱疹。病毒由上呼吸道进入人体，潜入脊神经后根神经节细胞或脑神经髓外神经节细胞中，平时不致病，但当过劳、患各种传染病、细胞免疫功能缺陷及长期使用皮质类固醇激素或免疫抑制剂之后机体免疫功能下降时，病毒繁殖而引发带状疱疹。病毒对组织有特异的亲嗜性，又只在细胞内寄生，病毒侵入皮肤后主要侵袭表皮，故病毒性皮肤病大部分表现为浅表皮肤病变。表皮细胞对病毒感染的反应有两种形式：一是炎症反应，引起表皮细胞变性及水疱形成，产生如麻疹、水痘样的皮疹；二是表皮细胞增生，形成各种疣。 病毒性皮肤病的临床表现主要有疣状增生、水疱形成和发疹等三种类型。常见的病毒性皮肤病及其致病的病毒见表。 有些病毒性皮肤病能引起持久的免疫力，如带状疱疹、麻疹等；有些则不能引起持久的免疫力，如单纯疱疹。有的病程缓慢,如寻常疣、尖锐湿疣、扁平疣等;有的则为急性过程，如麻疹、单纯疱疹、带状疱疹等。 病毒性皮肤病的诊断主要依据其临床表现，有的通过病理学检查、血清学检查及病毒分离等验证。 病毒性皮肤病的治疗因病种而异，有对症治疗、局部治疗及药物治疗三个方面。局部治疗是给以外用药、冷冻、电灼及切除等。抗病毒药物如吗啉呱、阿糖胞苷。无环鸟苷等可用于严重的原发型单纯疱疹及重症带状疱疹。值得注意的是，大部分病毒性疾病在确诊后或疑及本病时,一般不要使用免疫抑制剂及皮质类固醇激素,以免感染扩散。
