产后抑郁产后抑郁 是介于产妇郁闷和产后精神病之间的一种产后精神障碍。其发生原因与遗传因素、体内内分泌变化及生化改变、心理因素、家庭和环境因素有关。发生产后抑郁的危险因素有年龄偏大、既往有情绪异常史或精神障碍、孕期发生不良生活事件、孕产期得不到周围人及家人的帮助、不良产史、孕期出现躯体症状或孕产期保健质量差等。发病率报道不一致，为5%～30%。产后抑郁常发生在产后3周内或其后，其表现与一般抑郁症状相同，但症状常涉及婴儿、丈夫及家庭。主要表现为伤心落泪、内疚、焦虑、易怒、失眠，还可表现为精神压抑、无助感、沮丧、兴趣减低、悲观失望、自责自罪，甚至有自杀企图和他杀念头。2/3的患者可于1年内自行缓解，但1/3患者可遗留后遗症状。预防产后抑郁的发生是极为重要的，产前宣教应包括预防产后抑郁内容，尤其注重对孕妇特别对高危者及其家属的健康教育，产时产后使产妇能够得到丈夫，家庭和周围人的关心和帮助，减轻产妇生活中的应激压力。症状轻者可采用心理治疗，症状重者可服用抗抑郁药。
