足叶草酯毒素,疣脱欣, 鬼臼毒素药物名称:鬼臼毒素英文名:Podophyllotoxin别名: 足叶草酯毒素,疣脱欣, 鬼臼毒素外文名: Podophyllotoxin适应症: 尖锐湿疣、多发性浅表性上皮瘤病(如多发性浅表性或浸润性基底细胞上皮瘤,鳞状细胞上皮瘤和基底鳞状细胞上皮瘤),前上皮瘤性角化病,脂溢性角化、日光性角化和射线角化病,幼年喉头乳头瘤、疣(寻常疣、丝状疣)。 用量用法: (1)病变部位周围以凡士林保护,避免药液污染,或治疗部位涂药后立即以滑石粉撒布。(2)以牙签或玻璃棒蘸药液后,先滴一滴,干后再滴,使药液复盖病损,保留1~6小时,随后以肥皂和水洗净,如药液含安息香酊,以酒精棉球擦净,涂药后立即洗手。两次治疗间隔至少7天,可治疗4次,如仍无效,需更换其他疗法。通常应用浓度为25%,大面积损害(〉10~20cm2)应用5%~10%浓度。(3)对尖锐湿疣,使用10%~25%溶液,保持1~6小时,每周1次,共4次。(4)对多发性浅表性上皮瘤病或前上皮瘤性角化病,以25%溶液外涂,每日1次,连续数天。(5)对幼年喉头乳头瘤,外用以等量95%酒精稀释至12.5%,每日1次,当损害缩小时,可延长间隔涂药。 注意事项: 外用和误服可引起严重系统性毒性作用,通常是可逆的,但亦有致死的。口服本品300mg即可致死。大面积外涂、过量涂搽、较长时间涂用可发生严重毒性反应。本品涂在松脆、出血或新近活检疣的部位或将本品涂在病变部位周围的正常皮肤或粘膜,可增加系统中毒的危险。外用可发生肾功能衰竭和肝脏中毒(血清乳酸脱氢酶、门冬氨酸氨基转移酶和碱性磷酸增高)。神经系统反应发生较迟,持续时间较长,脑中毒可表现触性皮炎,系统性中毒的初起症状有腹疼或胃部疼痛、手脚不灵活觉、恶心、呕吐、腹泻,严重者或日久,可致白细胞及血小板减少,系统中毒的延缓症状有植物神经紊乱,如排尿困难、尿痛、头晕或头轻感(特别从坐位或卧位起立时),心跳加快、呼吸困难、倦睡、麻痹性肠梗阻(便秘、恶心、呕吐、胃痛)、周围神经病变(麻木 、刺痛、疼痛或手足软弱)、抽搐。本品可有交叉过敏反应,对安息香不耐吸收,能穿过胎盘,因有致畸作用,孕妇禁用。勿接触眼睛和粘膜。 规格: 鬼臼素溶液:25%。 类别:皮肤病用药
