妊娠期柯萨奇病毒感染妊娠期柯萨奇病毒感染  柯萨奇病毒属小RNA病毒科的肠道病毒属。大多数柯萨奇病毒引起的人类疾病表现轻微,但可引起许多不同的临床症候,同一病毒也可引起不同的疾病,柯萨奇病毒A16可引起手口足病。孕妇感染A组柯萨奇病毒可引起胎儿幽门狭窄、唇裂和腭裂。新生儿感染主要表现为胃肠炎和腹泻。孕妇感染B组柯萨奇病毒可引起胎儿先天性心脏病、泌尿生殖道畸形如尿道下裂及隐睾,由于病毒对血浆蛋白代谢的干扰作用可致胎儿宫内发育迟缓、胎儿肝脏及胰腺功能障碍。新生儿感染可出现消化不良、膈疝、糖尿病、急性心肌炎、肺炎及脑膜炎甚至胎死宫内或新生儿死亡。诊断:①孕妇有近期柯萨奇病毒感染的病史、症状(疱疹性咽峡炎和传染性胸痛)和体征。②病毒检测:a.标本:收集孕妇鼻咽部分泌物、大便及血液标本,或行绒毛活检、羊膜腔穿刺及脐带穿刺收集胎儿标本;b.检测方法:病毒分离、血清学检查、核酸杂交及多聚酶链反应。处理:孕妇柯萨奇病毒感染尤其是在早孕期感染时应作系统检查以排除胎儿畸形。对无胎儿畸形的孕妇按常规产科处理,应在条件较好的综合医院分娩。
