医患间的沟通医患间的沟通communication between the physician and patient  医患l’ed的沟通(eommunieation betweenthe Physieian and patient)从生物-心理一社会模式的角度来看,医师的主要职能是管理病人以减少疾病对病人的影响,而不仅仅是诊断和治疗疾病。因此临床工作者必须全面地考虑到病人的全部功能一一咱区体、心理和社会功能,这就要求医务人员善于同病人沟通。医患间的沟通发生于两种水平上。一种是技术水平上的沟通,医师对病人的疾病进行诊断和治疗。为此,医师必须向病人询问病史、为病人体检和安排实验室检查以及设计治疗方案。所有这些活动都需要病人的积极参与,都要求医患双方互相交流。为此,医师利用自己的技术性的医学知识,用病人易于理解的语言给病人以指导和解释。这种水平上沟通的基础是医师对医学的应用,故为技术水平上的沟通。另一种是非技术水平上的沟通,也就是说医患双方相互交往,就像卷入社会关系中的任何两个人一样地相互作用。两种水平的沟通是相互影响、相互依赖的,一种水平上的成功沟通有益于另一种水平上的沟通;从另一个角度看,一种水平上的相互作用或沟通的失败也会损害另一种水平上的相互作用。值得指出的是,现代医学科学技术的发展并不能减少医患间沟通的重要性。在医师的诊疗技能达到历史的最高发展水平的今天,医疗纠纷、诉讼案件不断增多,人们对医师的尊重和赞赏水平下降,其中的一个很重要的原因便是忽视了医患间的沟通,特别是非技术水平上的沟通。医患间的沟通方法可分为言语的和非言语的两大类。在诊断过程中,病人的非言语活动(如面部表情、姿势、步态等)往往为疾病的诊断提供线索。医师在同病人交往中的言语和非言语活动可以增进、也可以损害双方的沟通和医患关系,医师是医患沟通的主导方面。因此,要求医师提高自己的言语能力和会见病人的技巧,成为一个优秀的“倾听者”和思想情感的传送者。 (梁宝勇撰徐俊见审)
