黄色瘤黄色瘤xanthoma一种少见的脂类代谢性疾病。由于含有脂类的细胞在真皮或皮下组织内聚集，常在皮肤表面形成黄色的瘤状损害，故名。该病的皮疹形态和分布多种多样。主要有结节性、扁平性、发疹性及腱性4种类型。该病可以合并血脂代谢异常。有的病例有家族性，有的有系统性病变，如脂类沉积于心血管可产生动脉硬化和小血管的栓塞。有的还可合并或继发于其他疾病，如糖尿病、肝病、肾病等。当发现皮肤黄色瘤时，首先应确定其为单纯的皮肤型，还是高脂蛋白血症的皮肤表现。仅有皮肤损害而不侵犯内脏的病例，一般预后良好。常见黄色瘤主要有以下几种：①睑黄色瘤。皮肤损害为限局性柔软的小斑块，直径0.5～1厘米左右，境界清楚，稍高出于皮肤表面，对称分布于上眼睑内眦部，有时上下眼睑均可发病。多见于中年妇女。40岁以后发病者一般血脂蛋白正常。40岁以前发病且有家族史者，常伴有血胆固醇增高和早期动脉粥样硬化。②结节性黄色瘤。皮损为黄色或橘红色的丘疹和结节，分布于四肢伸侧关节处和易受压迫与摩擦的部位，如肘膝、髋、手足背、手掌等。粘膜也可受累，血脂蛋白增高，有时伴发胆汁性肝硬变、皮肤粘液水肿。③腱黄色瘤。附着在肌腱、韧带、筋膜，直径0.5～2.5厘米，质硬，好发于跟腱、手足背肌腱上，可随肌腱而活动，常有家族史。血清中脂蛋白增高，可导致心血管和肝胆的脂质沉积，而引起相应的系统性表现。④播散性黄色瘤。少见。⑤幼年性黄色肉芽肿。一种良性、自限性黄棕色丘疹或结节。常于出生后或生后一年内出现，多发性，12～18个月内自然消退。少数可以发生于眼、肺、心包、脑膜、肝、脾等处。治疗应首先查找类脂代谢障碍的原因，如饮食、遗传、糖尿病等，针对病因予以矫治。体重过高者应降低体重，调节膳食，进低脂肪、低热能、低胆固醇饮食，并应用降血脂药物。当血浆脂类降至正常时，几周内皮疹可以消退。大的结节可由外科切除，皮下损害可以电灼。
