旋毛虫病旋毛虫病trichinosis旋毛线虫寄生于人体引起的一种人畜共患的寄生虫病。假旋毛线虫也可引起。成虫寄生于小肠上部的粘膜上皮层内,雌虫受精后产卵,虫卵在其子宫内孵化为幼虫,幼虫排出后即钻入淋巴管或小静脉并随体循环到达全身各处,主要在横纹肌,尤其是膈肌、舌肌及其他活动较多的肌肉内成囊,其囊壁由宿主的组织反应形成,囊内幼虫可存活多年,但成虫寿命仅1个月左右。鼠、猪、犬、野猪、熊等均可为贮存宿主,人们常因进食不熟的带虫猪肉或其他肉而受感染。临床征象分为3期,肠期为幼虫在肠粘膜内发育为成虫,此时可有恶心、呕吐、腹痛及腹泻;移行期的症状有发冷、发热,体温与感染程度往往平行,颜面浮肿,全身肌肉肿胀和疼痛,以腓肠肌为明显,眼肌活动受限,咀嚼、吞咽、言语及呼吸困难,末梢血液酸性粒细胞数明显增多,也可出现心肌炎、脑炎、脑膜炎等征象,此期死亡率较高;感染后第3周幼虫周围开始形成包囊称为成囊期,此时发热等症状逐渐减轻,并进入恢复期。诊断主要依据吃生肉史、临床特征及血清免疫学反应阳性,肌肉活检也有助于确诊;检测循环抗原有助于早期诊断及考核疗效。大剂量丙硫苯咪唑治疗有效。
