士兵身份牌早在美国南北战争时，北军就在部队中开始配发了。当时的身份牌非常简陋，只是一个小纸牌，上面写着携带者所属部队的番号及本人姓名。作战中，若有人员伤亡，根据其身份牌上的记载，别人就可以知道伤亡者属于哪个部队。虽然这种身份牌质地粗糙，并且作用单一，但它却开创了世界军事史上军人佩带身份牌的先河。时光飞逝，转眼间第一次世界大战爆发了。战争打得非常残酷。由于新式武器的巨大威力，许多士兵受伤或阵亡后已血肉模糊，没法辨认。而那些极易损坏的纸质身份牌已不能适应军队的需要，于是金属身份牌便在战火中应运而生了。美军在一战中佩带的身份牌为圆形，由铝制成，在上面压印出佩戴者的部队番号、血型、姓名。当时，参战的各国军队基本上也都配发了金属身份牌，而美军在战争末期更是增加到每人两枚。二战时期，美军的身份牌又有了新的改进，首先，它的形状由圆形变为椭圆形，在材料上用铝合金取代了铝，使之更加坚实耐用。并且边缘处有一个小缺口。在残酷的战争环境下，许多尸体根本无法辨认，有的尸体更是被随便掩埋，当数年后被发现时早已成为了枯骨。而这时，美军的铝合金身份牌便发挥了重要的作用。当某人阵亡后，其战友会把阵亡者的两枚身份牌中的一枚放入死者嘴里，一枚放在尸体旁边。当死者无法被运回国时，也可以带走一枚身份牌当做死亡证明。但二战时的美军身份牌有一个缺点，就是各军种的形状都不一样，陆军偏小，陆战队与海、空军的大小也不一样，这给后勤保障带来了一定的困难。于是，到朝鲜战争爆发时，美军各兵种的身份牌都改为统一的形状及尺寸，同时边缘上的缺口也消失了，比起二战时的身份牌，这种统一的身份牌有了很大进步，特别是其材质采用不锈钢，比铝合金更加坚固耐用。越南战争期间，50年代研制的身份牌逐渐暴露出它的缺点。首先，越战时美军因为在热带丛林作战，所以在军服内穿的都是无领背心或索性什么都不穿，外面只穿一件热带战斗服。这样，不锈钢的身份牌便极易划伤裸露的皮肤。其次，这种身份牌在战斗中，特别是执行潜伏任务时，容易与武器碰撞，发出声响，暴露自己。由于以上的缺点，美国的地面部队及大部分海、空军士兵开始对这种身份牌深恶痛绝，许多人把这种被戏称为"狗牌"的小金属牌子扔在一边，取而代之的是一些宗教符号的标志或和平纪念章。针对这种情况，美国军方对这种身份牌又做了改进。先是在牌子四周包上一层塑料薄膜以改善其上述缺陷。这种方法虽然成本低，但由于塑料膜不结实，容易磨损，所以很快就不用了。很快地，一种新的改进方案被提了出来，就是在其四周套上橡胶圈。这样，不锈钢身份牌的上述缺点就不存在了，美国大兵们又把这些小牌子重新挂到了脖子上。美军现在使用的身份牌与越战时的区别不是很大，主要差异是在制作方法上。以前的身份牌上的铭文是采用机器压字的方法，成本高而且不漂亮。现在的美军身份牌上的铭文是采用光电技术来进行雕刻的，成本低而且美观大方。由于美国是个移民国家，所以在今天的美军身份牌上，除了常有的一些标识，如兵种、部队番号、姓名、军衔、血型外，还增加了宗教信仰一项，不同的宗教信仰用不同的字母来表示，如c代表信仰基督教（christianity ），b代表佛教（buddhism ），j代表犹太教（judaism ），l代表伊斯兰教（lslam）等，而无宗教信仰者则用nr表示。
