卵巢癌腹腔内化疗卵巢癌腹腔内化疗 腹膜吸收能力很强,可将化疗药物直接注入腹腔。根据药物动力学的研究,由于直接吸收,对残存在腹腔内的肿瘤细胞起直接杀灭作用,比全身用药的疗效好。注入化疗药物。穿刺位置必须正确,勿穿入肠腔或包裹性囊腔内。有腹水应尽量排尽,然后注入化疗药物;如无腹水,则可用生理盐水稀释药物,使药物能广泛分布于腹腔和腹膜表面。注入药物后,应尽量让病人在床上改变体位,使药物能达到腹膜各部位。腹腔内用药可以减轻腹水,而全身反应较轻,适于一般情况差,难以耐受全身化疗或腹水较多的病人。对腹腔化疗一直有不同观点,国内应用较多。在手术时将硅胶管安放在盆腔肿瘤附近,经腹壁引出,肠蠕动功能恢复后,可通过硅胶管注入药物。更好的方法是将药物泵埋于皮下,通过药物泵可反复给药。一般在30分钟内可输入2000ml液体,但有明显肠粘连、局限性包裹性囊肿、肠梗阻和腹膜炎时不应进行腹腔化疗。必须通过大循环吸收的药物,如环磷酰胺,通过肝脏分解酶的作用,才能发挥抗癌效力者,不宜直接用于腹腔内化疗。
