妊娠合并生殖器官沙眼衣原体感染妊娠合并生殖器官沙眼衣原体感染 沙眼衣原体是一种专性的细胞内微生物,分为15个血清型,引起沙眼(A、B、Ba及C型)、泌尿生殖器感染(D—K型)及性病淋巴肉芽肿(L1、L2、及L3型)。临床以子宫颈管炎最为常见,表现为黏液脓性白带、宫颈充血水肿、宫颈触血及宫颈管涂片白细胞增多。大约有一半的沙眼衣原体感染患者无临床症状,上行感染可引起子宫内膜炎及输卵管炎等。孕妇沙眼衣原体感染可引起胎死宫内、出生低体重儿、早产及新生儿沙眼衣原体感染。在普通人群中,婴儿沙眼衣原体感染率为1%~5%,但在孕期沙眼衣原体感染母亲所生婴儿中沙眼衣原体感染率高达70%,其中20%为肺炎。对孕妇应用抗生素治疗后,新生儿沙眼衣原体感染率可降低至5%。无并发症子宫颈治疗选择以下方案之一:①红霉素500mg,口服,每日4次,连用7日;②红霉素250mg,口服,每日4次,连用14日;③羟氨苄青霉素300mg,口服,每日3次,连用7日;④氯洁霉素300mg,口服,每日4次,连用7日。治疗期间应避免性生活,同时对配偶进行治疗,治愈后在孕36周时再次复查,以免复发。
