钙代谢紊乱  钙的吸收、排泄或体内分布的异常。临床常表现为高钙血症、低钙血症、并可影响骨的代谢和发育,引起佝偻病、骨软化症等骨代谢病。钙代谢与磷代谢密切相关,与体内酸碱平衡也关系密切,这几方面的异常可以互相影响。    钙是人体内含量最高的元素之一,含量居第8位。钙离子是人体内含量最多的阳离子,是骨组织的主要成分之一,也在机体各种生理和生物化学过程中发挥重要作用,体内总钙量的99%分布在骨骼,0.5%在牙、0.5%在软组织,0.02%在血浆。骨中的钙多为磷酸盐:Ca3(PO4)2及Ca3(PO4)6(OH)2等。血浆中的钙有3种形式:48%为离子化的钙,有生理活性;46%为蛋白结合钙(钙与血浆蛋白结合),血浆白蛋白降低时,血浆总钙量下降,但游离钙可不受影响;与有机酸(如柠檬酸)或无机酸(如硫酸、磷酸)结合的钙为复合钙。总钙量不能反映游离钙的水平。血中[Ca2+][HPO厈][HCO婣]的乘积与[H+]的商为一个常数:[Ca2+][HPO厈][HCO婣]/[H+]=常数。[Ca2+]与[H+]成正比,故酸中毒时,血中总钙量即使下降,游离钙亦可升高,碱中毒时则降低。    钙的生理功能为:①骨的主要成分。②调整神经肌肉的兴奋性,血钙降低时神经肌肉应激性增高,可致手足搐搦乃至全身抽搐。③影响肾上腺素,甲状旁腺激素,胰岛素、高血糖素及胰液,唾液的分泌。④激活许多酶,如淀粉酶、脂酶、 胰蛋白酶碱性磷酸酶、凝血酶原等。⑤激活补体。⑥激活纤维蛋白溶解作用。⑦激活凝血因子(见止血机制)。⑧使血小板凝集。⑨维持血管上皮的完整性。⑩重要的第二信使(见第二信使)。    成人每日需钙0.62kg,儿童、孕妇及哺乳期妇女需要 1.5~2.0g。大部分食物中均含不同量的钙、深绿色的蔬菜、连骨连壳吃的小鱼小虾、豆类,骨粉含钙较多,奶及奶制品中的钙吸收率高。    钙在小肠吸收,这是一个逆着浓度梯度的运载过程,需要能量。1,25-二羟维生素D3促进肠壁合成钙结合蛋白,钙结合蛋白在钠泵或钙泵作用下,促进钙的吸收。故维生素D缺乏可影响钙的吸收。甲状旁腺激素,胆酸、含柠檬酸多的食粉,食物中钙磷比率高,十二松胶pH低,都有利于钙的吸收;食物中多含植酸盐、草酸盐、磷酸盐,肾上腺糖皮质激素,降钙素小肠pH值高,均妨碍钙的吸收。    钙主要从尿排出,从肾小球滤过的钙离子99%从近曲小管重吸收,钙重吸收常伴钠的重吸收。利尿和排钠同时也伴排钙。噻嗪类利尿药(如双氢氯噻嗪)抑制钙的排泄。甲状腺滤泡旁细胞分泌的降钙素以及肾上腺糖皮质激素可抑制钙的重吸收,而25-羟维生素D3及1,25-二羟维生素D3可增加近曲小管中钙、磷的重吸收。    血总钙浓度为2.1~2.7mmol/L(8.4~10.8mg/dl),游离钙为1.07~1.27mm0l/L(4.3~ 5.1mg/dl)。正常情况下,血钙、血磷浓度(mg/dl)的乘积为一常数(35~40)血钙数值与钙的吸收,排泄及骨钙与循环钙之间的动态平衡有关。主要的影响因素是甲状旁腺激素(动员骨钙、抑制钙在近曲小管的重吸收,增加远曲小管的钙重吸收,促进肠吸收,增加1,25-二羟维生素D3的合成),降钙素(抑制骨质吸收,促进成骨,抑制钙在肾曲小管的重吸收,抑制肠钙吸收)和维生素D(促进肠钙吸收,增加骨再吸收,促进骨盐沉着,增加肾近曲小管对钙的重吸收)。因此甲状旁腺功能亢进则导致高钙血症、低磷酸盐血症和血碱性磷酸酶过高症。维生素 D缺乏可影响钙、磷吸收,致低钙血症、佝偻病、骨软化症等。低血钙性抗维生素D性佝偻病是肾脏缺乏1-羟化酶,血钙低,血磷正常或稍低,大剂量维生素D治疗有效。肾性骨病又称肾性佝偻病,因肾功能衰竭致钙磷代谢障碍,血钙降低,血磷增高,骨痛,行走困难,乃至畸形。
