脓疱疮 皮肤上遍发水疱、壁薄而透明、易腐烂溃破为临床特征的常见化脓性皮肤病。又名黄水疮、天疱疮。具有接触传染及自身接种的特性，多见于夏秋之季，在2～6岁儿童中流行。脓疱疮病变仅在表皮，若及时治疗可不留疤痕。若未能及时治疗，少数病人可并发肾炎、肺炎。新生儿脓疱疮有可能并发败血症。 古代对脓疱疮早有认识，并指出能自发传染及相互传染。宋代《疮疡经验全书》说，此疮之发，合家相染。清代《洞天奥旨》也指出其脓水流到之处，即会生疮。 脓疱疮多因感受暑湿热毒，以致气机不畅、疏泄障碍，熏蒸皮肤而成。小儿体质虚弱、皮肤细嫩、汗多湿重、暑邪湿毒更易侵袭而致病。脓疱疮多发于头面四肢等暴露部位，也可蔓延全身。皮损初起为红斑或水疱，约黄豆至豌豆大小，迅速变为脓疱，界限分明(四周有轻度红晕)，疱壁极薄，容易破裂，显出湿润而潮红的疮面，流出黄水，干燥后结成脓痂，痂皮逐渐脱落而愈。皮损初起感瘙痒，破后糜烂时疼痛。一般无全身不适或有轻度发热；严重者可有高热、形寒，面目浮肿，尿少等证。新生儿患脓疱疮，因抵抗力弱，病情重且易产生并发症而致生命危险。此病一般1～2周痊愈。若治疗不及时或治疗不当，可绵延数周或数月。 临床常见证型有：①湿热型：脓疱较多、密集、色黄，周围有红晕，破后糜烂而鲜红，附近核肿大，或伴发热，口干，尿黄，便燥，苔黄腻舌质红，脉濡数等。治宜清暑利湿、清热解毒，用清暑饮加减。②脾虚型：脓疱稀疏，色淡黄，周围红晕不显，破后糜烂面淡红，或伴有面色??白或萎黄，纳呆神倦，便溏，苔薄舌淡，脉濡细。治宜健脾渗湿，用参苓白术散加减。 外治：脓疱疮主要以外治为主。①青黛散外扑或用麻油调成糊状外搽，每日二三次。②用颠倒散洗剂外搽，每日四五次。③糜烂结脓痂较厚者用5%硫黄软膏外敷。 护理与预防：①病变处禁用水洗，如欲清洗脓痂，可用10%黄柏溶液擦洗。②炎夏季节每日洗澡一二次，浴后扑痱子粉，保持皮肤干燥。③病变部位应避免搔抓防止传播。④幼儿园、托儿所在夏季要对儿童每日检查，发现患儿，立即隔离治疗，患儿接触过的物品应及时消毒。
