儿童精神障碍儿童精神障碍childhood,mentaldisorderin发生于儿童和少年期(18岁以前)的各种行为异常和精神疾病。包括:①儿童行为障碍(见儿童行为和情绪障碍)。如遗尿、遗粪、口吃、偏食、厌食、贪食、异食、失眠、恶梦、夜惊、睡行、吮手指、习惯性抽动等。②儿童品行障碍。③多动综合征。④儿童情绪障碍。如过分害羞、恐惧、焦虑、暴怒发作等。⑤特殊功能发育障碍。如构音障碍、言语表达障碍、言语理解障碍、阅读障碍、拼音障碍、算术能力障碍、运动技能障碍。⑥儿童神经症。如分离性焦虑、焦虑症、恐怖症、强迫症、癔病。⑦儿童精神病。如儿童孤独症、躯体性精神病、精神分裂症、躁狂抑郁症。⑧精神发育迟滞。儿童不是成人的简单雏形,儿童精神障碍有其自身的特点。①发生原因方面,环境因素的影响超过成人,是重要的诱因之一。儿童品行障碍、行为障碍、多动综合征、情绪障碍和神经症的发生,与家庭及社会环境不良、教育失当有关。②症状方面,儿童精神障碍的表现或为单一的行为障碍、情绪障碍,或为单调的精神症状,妄想少见,病理性幻想多见;语言性幻觉少见,形象性幻觉多见;思维症状少见,运动性症状多见。③预后方面,一般来说,儿童行为障碍、情绪障碍、多动综合征、特殊功能发育障碍和儿童神经症预后较好;品行障碍预后较差,其中有的到成人后容易成为反社会型人格,存在犯罪问题。大多数精神发育迟滞的儿童经过教育训练以后,能力可有不同程度的增长。儿童精神病普遍预后不良。④临床实践方面,儿童的言语表达能力较差,精神检查主要是观察儿童的行为,其次是通过交谈。判断儿童行为是否异常,要考虑年龄因素,如3岁以内的儿童尿床是正常现象,7岁儿童还尿床就不正常了。⑤治疗方面,首先要改善环境、家庭气氛和教育方法,消除不良影响;心理治疗应以启发、诱导和鼓励为主;行为治疗对行为障碍、情绪障碍和部分神经症有效;药物治疗主要用于多动综合征和儿童精神病。
