腭裂  面颌发育过程中形成的先天畸形,腭部留有裂隙。俗称狼咽。影响美观、吸吮和发音,可与唇裂合并发生。手术治疗有效。    发病  在胚胎发育初期,原始口腔未分化出口腔和鼻腔,以后两侧上颌突各突出一块板状突起(侧腭突),向中线生长,融合形成腭,将口腔与鼻腔分开,若融合不全即成腭裂。    发病原因未明,可能与营养、遗传、感染、损伤、内分泌、药物等因素有关(见唇裂)。    临床表现  腭裂的部位可发生在软腭和悬雍垂,也可贯通软腭、硬腭乃至鼻孔,与唇裂相连。由于鼻腔与口腔相通,患儿在哺乳期可有不同程度的吸吮功能障碍,常须用匙喂食,进食时,食物可能进入鼻腔,冷空气及尘土也可能不经鼻腔过滤、加温而直接进入咽部,故常易引起上呼吸道感染。正常人发某些音时,软腭上提与咽周围及后壁形成腭咽闭合。而腭裂病人无法形成闭合,不能有效地控制口腔中的气压及气流,空气大部分从鼻腔漏出,因此,在讲话时就不能角正常人一样发出清晰的语音,而代之以一种特殊的、带有鼻音的、难以听懂的腭裂音质。    治疗  有效的手术治疗是在19世纪后期开始的。腭裂的综合治疗小组一般包括口腔外科、成型科、耳鼻咽喉科、儿科、心理学、 X射线学、口腔正畸、口腔矫形及语音病理学等多方面的专家和医务人员。一般在患儿出生 4周后进行术前的正畸(关闭裂隙),在12~14个月行腭裂修复术。术后进行吸吮、吹气等训练,从3岁开始进行语音训练,待8岁左右决定是否做第2次手术。而唇部、鼻部的第 2次修复手术在10岁以后进行。通过一系列的治疗,患者可达到几乎同正常人一样的发音水平。    若腭裂患儿在幼年未得到及时治疗,成年后也可治疗,手术在局部麻醉下进行,瘢痕比幼年时手术明显,虽经治疗后腭咽闭合良好,但因患者多年发音习惯已养成,仍不免常有鼻音。
